Here is the reason from 2011 and the same happened in 2012. I can only see the problems as being deliberate on CreateSpace's site because I had files doublechecked by people who used the company and there were no problems.
So, here you go:

I could really get paranoid that “someone” is trying to stop me publishing! :-/
These books have all be printed by lulu.com with no problem. They look perfect. Even the Indian POD has printed them with no problems.
I ALWAYS leave a bleed around the cover –its something I keep telling my brother if he is putting one together:ALWAYS leave space around the edges.
All covers are around 300dpi+ so the idea of a cover at 227 dpi is ridiculous. I have no idea what they are talking about!!
I have checked all the interior pages and every single one is 300 dpi. Not a single 156 dpi any where. I mean, for ***** sake I have to resize and then double check the pdf files several times to guarantee no errors.
ALL the interior file pdf pages are sized the same with no exceptions. In fact, when an error did occur on one book the Indian POD printer sent a message that they had “automatically resized the pages” and sent me the pdf to see that it looked okay.
Seriously, I just have no idea what’s going on –I had a friend at a printers check the pdf referred to and he just asked “What’s supposed to be the problem?”
Anyway, here are the messages from Create Space.
Before your book can be printed, you need to address the following:
PROBLEM:
The cover file is lacking bleed on the right, left sides. The cover must have at least .125 bleed on all sides. You may wish to review the Artwork and Templates section, available at https://www.createspace.com/Help/Book/Artwork.do.
PROBLEM:
The cover file contains live elements that may be trimmed during the production process. Please make sure that all text appears .375 away from the outer edges. Additionally, all elements you wish to appear on the cover, such as text and graphics, need to appear within the live graphics area. Only background that can be cut off should extend through the bleed area.
NOTED:
The cover file contains images at 227 DPI, which may appear blurry and pixelated in print. For optimal printing, we recommend all images be at least 300 DPI.

The interior file submitted was too large to fit the selected trim size; therefore, we have adjusted the file to ensure that no live elements are cut during the manufacture process. This adjustment may have impacted the font and/or margin size. We recommend reviewing a proof copy of your book and adjusting the native file if any changes are desired. You may wish to use our interior templates for standard trim sizes that are available for black & white books without bleed. Interior files should be formatted to the exact final trim size of your book and submitted as a single PDF file with one text page per PDF page. You may wish to use our interior templates for standard trim sizes, available for black & white books without bleed. Well, apparently it contains “Color pages that will be printed in black and white”
So the response I got does not refer in any way to image size but MARGIN sizes and I leave margins around all pages and covers!!! Its basically, we will not print your book because we want all of them re-scanned/re-sized and we don't care if the booksprint perfectly fine with other services!!!

We strive to produce quality products for our members and their potential customers. For this reason, we are unable to waive our submission requirements. While we allow our members to maintain the majority of the creative control, we are unable to sway from these specifications.
Text and images intended to appear on a printed version of a book, also referred to as "live elements," must be at least .25" inside the trim lines to ensure they are not cut during the bookmaking process. This is referred to as our "Safe Zone" requirement. The intent of this measurement is to ensure all live elements appear within the desired area within a printed book.
If any live element falls within the .25" requirement, we cannot guarantee it will appear within the printed version of a book and all files will be paused during the review process as a result.
In regard to bleed, Your cover's background must extend at least .125" beyond the final trim line to accommodate for cutting/trimming tolerance.
